Experience: None required

Eligibility: Audio-Visual Equipment Operator/Technician (MC No. 10, S.2013-Category II)

Place of Assignment: Miagao Interactive Learning Center-Teaching and Learning Resource Center, UP Visayas, Miagao, Iloilo

1. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) and Work Experience Sheet (WES) which can be downloaded at www.csc.gov.ph;
2. Photocopy of Highest Educational Attainment issued by the Department of Education/CHED or Photocopy of Transcript of Records and Diploma issued by the University Registrar;
3. Photocopy of Certificate of Eligibility as Audio-Visual Equipment Operator/Technician issued by the Civil Service Commission;
4. Certificate of employment issued by HRMO or authorized official (only for work experience relevant to the job);
5. Certificate of performance ratings obtained in the last two (2) rating periods (January to June 2021 and July to December 2021);
6. Certificate of attendance to trainings, seminars and workshops for the last five (5) years (July 2017 to June 2022);
7. Certificate of Accomplishments (Awards/Citations/Innovations/Critical Incidents)
